I – Permit for the transportation of human ashes (remains) to Viet Nam
Documents to be submitted to the Consulate:
1. Application for a Permit for the Transportation of Human Ashes to Vietnam (Form M03).
2. Permission from the People’s Committee at sub-district level in Vietnam where ashes will be transported to (Form M04). A family member in Viet Nam should fill out the attached form, submit it to the local authorities for endorsement, and send it back to the US-based applicant for submission to the Consulate.
3. Photocopy of Death Certificate and the Certificate of Cremation.
4. Photocopy of Passport or Green Card of the late person.
5. Photocopy of the Permit for Transit of Human Ashes issued by the local (State/County) Department of Health.
6. Statement of No Infectious and Contagious Diseases for the human ashes issued by the local (County/State) Department of Health.
7. Self-stamped and addressed return envelope (if applicants want to receive visa by mail). To avoid losses during shipment, it is advised to use guaranteed services of USPS / UPS or Fedex with tracking number for your easy tracking. The Consulate General shall not be responsible for any missing documents due to your choice of using uncertified mails.
8. Fee: Please pay by money order, cash or personal check to the “Consulate of Viet Nam.”
Processing time: On the next working day as from the date of receipt of the application and all supporting documents, the Consulate will issue and send back the permit.
II – Permit for the transportation of a dead bodies of Vietnamese Origin to Viet Nam
Documents to be submitted to the Consulate:
1. Application for a Permit for the Transportation of Dead body to Vietnam (Form M01).
3. Photocopy of Death Certificate.
4. Photocopy of Passport or Green Card of the late person.
5. Photocopy of the Permit for Burial/Transit of Dead body issued by the US local (State/County) Department of Health.
6. Statement of No Infectious and Contagious Diseases for the dead body issued by the local (County/State) Department of Health.
7. Self-stamped and addressed return envelope (if applicants want to receive visa by mail). To avoid losses during shipment, it is advised to use guaranteed services of USPS / UPS or Fedex with tracking number for your easy tracking.
